How to Register

Are you going to join us for this great day?  FANTASTIC!  Here is what you need to do:

1.  Fill out the registration form to reserve your space.  You will need to download the form to your computer and fill out the requested information.  Save the completed document to your computer.

2.  Decide how you want to pay. 

• Credit Card payments can be by clicking the 'Buy it Now' button below until May 15th.  We are not able to accept credit card payments at the event.

• Check or Mail Orders made out the the American Cancer Society, can be mailed with your completed registration form to: Kristin Tierney, 5330 Bressler Drive, Hilliard, Ohio 43026.  Your envelope must be post marked by May 15th for the discounted rate.
• Only cash and checks will be accepted at the event.

Pre-registration with payment by May 15th gives you a $10 discount!  A valid credit card number or pre-payment must be provided at the time of your registration to hold your space.

3.  E-mail the completed form, as an attachment file, along with payment choice to krzykramer@yahoo.com

4.  Mark your calendar: May 22nd, 9am-9pm

That's it!  An e-mail conformation will be sent with-in 24 hours.
